COM_INIT_DB isn't routed to all shards

A COM_INIT_DB command is only routed to a single backend that uses the database. If more than one shard contains the database in question, the default database of the remaining ones is left unmodified. Defining the default database while the connection is being created still works as expected, only a change of database while the connection is open suffers from this problem.
