Document change in behavior of mysql.host and blank host field in mysql.db tables

    Description

      The mysql.host table is still used in MariaDB 5.5 if the mysql.db host field is blank. At some point in MariaDB 10.0, the meaning of a blank mysql.db host field changed, and the host table became unused.
